The permit to work is a written system that aims to ensure that proper procedures are carried out and no mistakes or misinterpretations of instruction take place

Before any work starts potential hazards must be identified and eliminated

Risk assessment to be carried out

All work to proceed as per COSWP.

Before work can start on high voltage switchboard the responsible person must

Switch off the equipment, lock off, isolate and earth.

Test to be done that equipment is dead and safe for work

State where equipment is earthed.

Put warning notice.

Sign, date and issue a permit to work to workman.

Workman must sign to accept permit to work before starting work

Responsible person must ensure, sign and cancel permit to work and recommision equipment.

Work done must be entered in engine room log book and the PMS paper work must be completed.

The permit to work should be filed and kept for future reference.
